Yellow Fever Certificate Reissue

Description

If you have previously been vaccinated but have lost your yellow fever certificate we can issue a new one to you provided that you have evidence of previous vaccination.

You will need to provide a scanned copy of the following:

Date of Vaccination
Batch number
Name of vaccine
Proof of ID

Note:

Upon booking an appointment you will receive an email with a link to a form that must be completed before the appointment to ensure you have the correct details for us to reissue the certificate.

Furthermore, Any cancellation or rescheduling within 48 hours before the appointment will incur a charge of 20% of the total service cost and missed appointments will be deemed non-refundable.
